to Investigate an Incident
When
an incident happens, it is important on the part of the organization to record
it correctly and investigate it to avoid recurrence.
Incident
Investigation helps in identifying the root cause of the incident and with
suitable control measures; it can be avoided in the future.
Before
starting the interview make sure that it is an appropriate time to interview the
witnesses. Some witnesses may be in shock and may not be able to respond
promptly.
Interview
the witnesses in a quiet place, mostly away from the incident area to have a
proper interaction with minimal distractions.
Interviewer
should introduce themselves and explain the purpose of the interview clearly to
the witnesses.
Interview
should be a fact-finding mission rather than a fault finding mission. The main
objective of the interview is to find out the cause of the incident and avoid
it happening in the future.
Interviewer
should use open-ended questions like "What", "Where",
"Why" etc. This will allow the witnesses to openly express their
opinion.
Interviewer
should keep an open mind and not jump into any conclusions before analyzing.
The interview should be recorded and signed at the end of the session as a
record.
Conclude the interview by thanking the witnesses
